Initial Survey

Weekly Surveys
Please complete one weekly survey at the end of each week of LifeLine app use.
Week 1 Survey
Complete this survey after your first week of using the LifeLine app.
Week 2 Survey
Complete this survey after your second week of using the LifeLine app.

Exit Survey
